Alice Braga plays Anna, a survivor who arrives and disrupts the solitary routine Neville has built around his obsession with finding a cure.

The theatrical ending drew backlash for diverging from Matheson's source material; an alternate cut exists that restores the book's core idea and reframes what the title actually means.
