Three decades after the rage virus swept Britain, the infected are still out there — and something may have changed in them. Dr. Kelson (Fiennes) keeps an enormous alpha sedated in a facility and starts to notice what looks like awareness behind its eyes. Miles away, a teenager named Spike (Williams) stumbles into the orbit of Sir Jimmy Crystal (O'Connell), a charismatic cult leader who runs a band of survivors-turned-killers. Escaping the Jimmies turns out to be as lethal as facing an infected.
