The owner of a nearby villa, retired surgeon Dr. Heiter played by Dieter Laser, invites them in.

Heiter built his career separating conjoined twins. His new obsession runs the other direction. He plans to surgically connect three captives into a single organism sharing one digestive tract. By morning the two women understand the full scope of what he intends — and that the police are nowhere near finding them.
